The DOD sends two letters to Iraq, one in English and one in Arabic. Both say they are withdrawing US troops, but portions of the Arabic letter contradict the English letter. So Iraq asks for a clarification and is given a newly translated letter that aligns with the English version. Then the US announces the letters were mistakes and never should have been sent.

Iraq, meanwhile, is understandably wondering what they believe and what to rely on.

No clue what caused the problem, but it's really a bad look for the US and speaks to a chain of command that is disjointed or inadequate.
